SHIPPING
1. Orders for “blank” forms are normally shipped within 2 to 3 weeks after receipt of order. No forms are shipped until final 2004 forms are approved by the IRS. This approval is normally around September 1st.
2. Examine all shipments of imprinted order forms for accuracy immediately upon receipt. Please contact Kraftbilt if you are not satisfied with your order. No returns will be accepted without prior authorization.
3. Imprinted forms are usually shipped within 2 to 4 weeks after receipt of order. (Delivery time may vary in November and December.) Order early to avoid delays!
4. Imprinted orders must be ordered by mail or fax in order to insure accuracy. Sorry, no imprinted orders accepted after December 1.
5. All prices are F.O.B. plant with cost of shipping added to invoice.
6. Tax form samples are available upon request. Please contact Kraftbilt Toll-free: 1-800-331-7290. Mail orders to Kraftbilt, P.O Box 800, Tulsa, Oklahoma 74101.

ORDER INFORMATION
1. Complete the order form-please include street address for FedEx delivery, and P.O. box for billing.
2. Minimum order is a total of 50 forms.
3. For imprint orders, please list full name, phone, number, federal, social security, and/or state identification numbers as they appear on tax rolls. Imprinting charge is $18 per thousand or any fraction thereof.
4. Special state imprint (the 2-character state name abbreviation placed in the state name box) on forms is not required by either the IRS or the individual states. However, if a state imprint is listed on your Tax Form Order Card, the charge for imprint will be $7.50.
5. All forms on the website are available, even if they are not priced. For additional pricing or information, just call toll-free: 1-800-331-7290. There will be no returns after December 1st.

If you are not completely satisfied with your tax form order, please notify Kraftbilt within 30 days of receipt and we will replace your order or refund your purchase price. No returns accepted without prior authorization.
